Yeah, well on the flip side of that coin we look at operator error.

Back in the early days I miswired a generator. I got it all set up and started the diesel. I then went into the other room and flipped the switch. The engine immediately went into survivor mode trying to maintain RPM. I poked my head into the engine room to see the diesel exhausting a thick cloud of black smoke but gamely trying to maintain RPM.

And of course the generator was also emitting a thick cloud of smoke from being shorted. It totally ruined my day.
